The world was still deep in the Great Depression when William Kaufman, MD, PhD, had already begun treating osteoarthritis with two to four grams of niacinamide daily. Now, over 60 years later, his pioneering work in orthomolecular medicine is receiving the recognition it so well deserves.
In a 1978 radio interview with Carlton Fredericks, Dr Kaufman told of how "I had one patient who was so severely arthritic that I could not bend his elbows enough to measure his blood pressure. He was one of my first patients. I gave him niacinamide for a week in divided doses, and then he could bend his arm. I took him off it and gave him a look-alike medicine (placebo). In a week he was back where he was before: his joints were stiff again.
"I arrived at my (megavitamin B-3 dosage) schedule by actually seeing the response of patients with varying degrees of arthritis. One cannot give a single large dose and get any really favorable results in arthritis... It is necessary to divide the doses so that the blood levels of niacinamide would be fairly uniform throughout the waking day."
Kaufman's findings were both plain and elegant. The greater the stiffness, the more frequent the doses. Severely crippled arthritic patients needed up to a total of 4,000 mg/day. Divided into 10 doses per day, in one to three months, patients could now get out of their chair, or bed. "If continued, they would be able comb their hair and be able to walk upstairs, so they would no longer be prisoners of the house. By the end of about three years’ treatment, they would be fully ambulatory, and this was even in the older age groups."

The authors of a 1996 study on niacinamide and osteoarthritis (Jonas WB, Rapoza CP, Blair WF. The effect of niacinamide in osteoarthritis: a pilot study. Inflammatory Research 45:330–334.) could have omitted the words "pilot study" from their title. Dr. William Kaufman had already published, 47 years earlier, his meticulous case notes for hundreds of patients, along with specific niacinamide dosage information applicable to both osteoarthritis and rheumatoid arthritis.
In addition, the doctor added some remarkably prescient observations on the antidepressant-antipsychotic properties of B-3.
Dr. Kaufman, whom his widow has described as a conservative physician, was nevertheless the first to prescribe as much as 5,000 mg niacinamide daily, in many divided doses, to improve range of joint motion.

Avoiding Joint Replacement Therapy by Eliminating Osteoarthritis Pain: The Research of William Kaufman, PhD, MD – ANH Newsletter Archive

Dr. William Kaufman in the 1940s thoroughly researched and proved a safe, effective remedy for osteoarthritis pain. This remedy also improved joint mobility significantly.
• This in the days before bread / white flour products were “enriched”.
• I.e., patients were starting from a position of real deficiency or semi-deficiency.
• 1943 Dr. Kaufman published a book describing the mental and physical effects of a vitamin deficiency of niacinamide in 150 patients in the prior year.
• [Interview]:
• Each patient remained in the office for at least an hour, so I could observe what happened.
• Within 2½ to 5 minutes after taking the first 100 milligrams of niacinamide there was a degree of physical and mental relaxation which became marked in the next 20 minutes.
• The first objective change, apparent within the first 5 minutes, is the relaxation of previously tense muscles, and the replacement of a drawn facial expression by a more calm one, or even a smile.
• Without suggestion, patients began to sit, walk and stand more erectly.
• Within the first 5 to 10 minutes, the color of the hands and feet might change from a sallow yellow to a healthy pinkish or ruddy color, and the hands and feet are frequently subjectively and objectively warmer.

An amazing and fun story of healing … with “just” nutrition.
If Dracula were an old woman, he would have sounded a lot like Mrs. Kelremor. But there was no other physical resemblance. Just shy of eighty, this middle-European immigrant lady had been a housekeeper for decades. Overweight, bent over, worn out and weighted down with cares, she came to see me primarily because of osteoarthritis.
It was the 1980's: disco was still considered music, and arthritis was still said to have no nutritional connection or cure. For centuries, natural healing practitioners have known otherwise. Today, more and more, the medical profession is finally catching on. But Mrs. Kelremor had waited long enough. She said to me [Andrew Saul], in her thick Transylvanian accent:
"I can't vork. I can't svleep. I om in pain all oof da time.”
[ healing program … "I will try anything.” Anything? Even living on raw vegetable juices eight days in a row, followed by a very light eating for three days, and a raw-food diet for the next ten? ]
. . .
A year passed.
One day I [Andrew Saul] was shopping in a friend's health food store. There were a few people at the check-out counter. One was a tallish lady, or if not tall, she certainly had very good posture.
"Remember me?" she said, with the unmistakable voice of Bela Legosi on estrogen.
I recalled only the voice. It did not match this graceful woman, at ease and smiling, buying a counter full of vitamins. But it was Mrs. Kelremor.
I greeted her, and she wasted no time in telling me:
"I can work. I can bend, and reach, and sit and stand, and walk without any pain. I can work! I feel like a new woman."
She actually said something more like, "I veel loke new voman,” ...
I couldn't help but notice that the lumps on her arms and legs were gone. It wasn't surgery; a year and a half of juicing had apparently eradicated them. Now that was a bit unexpected.
And all this progress, past age 80. …
